Understanding Dysmenorrhea
Dysmenorrhea is severe abdominal and uterine pain with menstruation.
Dysmenorrhea affects nearly 3 in every 4 women at some time during their reproductive life. In most women the pain is uncomfortable and 30% of women need to take some form of pain killer. At the more severe end of the spectrum dysmenorrhea limits the daily activity in 15% of women and 8% miss work or school. It is an increase in hormone-like substances called prostaglandins prior to bleeding that cause the uterine muscle to contract and give rise to the pain associated with menstrual bleeding. Menstrual blood flow is mostly controlled by the hormones estrogen and progesterone. Progesterone opposes the action of prostaglandins whereas estrogen increases prostaglandin production. There is a fine balance between the two hormones and if there is a disruption to one or both hormones, often the disruption can be reflected in menstrual bleeding changes and dysmenorrhea. Most women who experience debilitating and cyclical dysmenorrhea are progesterone deficient.
